PETITION TO GRADUATE FORM
 

PETITION TO GRADUATE/ONE-YEAR CERTIFICATE - Each student who wishes to graduate or receive a one-year (50 credit hours or more) certificate must obtain a “Petition to Graduate/Certificate” form from the Gateway Student Records Office TWO and ˝ QUARTERS IN ADVANCE of the quarter in which the student intends to complete requirements.  The student is to meet with his/her advisor for an evaluation of all course work completed, review of cumulative grade point average, and review of courses for which he/she plans to complete within the next two quarters to determine eligibility for graduation.  The petition must be approved by the student's academic advisor and filed with the Gateway Student Records Office by the published deadline date for the quarter of completion.

GRADUATION/CERTIFICATE REQUIREMENTS

In order for a student to be considered a candidate for an associate degree or one-year certificate, he/she must: 

1)    Satisfactorily complete the prescribed curriculum leading to that degree or certificate.

2)    Attain a minimum cumulative grade point average of 2.0.

3)    Complete the petition to graduate/certificate process by established deadline dates.

4)    Must be officially registered in a class with COTC during the final quarter of degree/certificate completion.  Proficiency credit, Non-Traditional credit, and Transfer credit do not apply toward meeting this requirement.

NOTE:  Courses which the student expects to transfer must be clearly indicated on the "Petition to Graduate/Certificate" form. An official transcript from the other college must be received before a diploma/certificate or COTC transcript will be issued.

COMMENCEMENT - Graduation ceremonies are held in June and December with the conferring of degrees and certificates.  Any outstanding financial obligations must be satisfied prior to participating in either ceremony.  Any student who has received financial aid must also complete an Exit Interview Survey. A student who completes requirements at the end of Summer or Autumn Quarters is invited to participate in the December ceremony.  If they prefer, a student may elect to wait and participate in the June ceremony.  A student who completes requirements at the end of Winter or Spring Quarters is invited to participate in the June ceremony.

ADDITIONAL NOTES:

·         A student making a name change following submission of the “Petition to Graduate” form must notify the Gateway Student Records Office.  In cases where the diploma order has been processed, an additional fee may be required.

·         If changes are made to the Petition to Graduate, students are responsible for keeping the Records Office informed of any changes.
